| Impact statement: This case report documents the first recorded instance in Bangladesh of an adult with Down syndrome experiencing "long COVID"—specifically, persistent muscle and joint pain lasting more than 12 weeks after initial infection. The study explains that individuals with trisomy 21 are biologically predisposed to severe COVID-19 outcomes due to immune dysregulation and interferon hyperactivity, which can trigger a massive cytokine storm. By establishing that mortality risks for this population begin to rise significantly at age 40, much younger than the general public, the research provides a vital evidence-based mandate for health authorities to prioritise people with Down syndrome for early vaccination and to update monitoring standards to include annual musculoskeletal examinations. | Collaboration: None | Keywords: Down syndrome, COVID-19, Long COVID, Trisomy 21, Bangladesh, Case report |
