Impact statement: This cross-sectional study of 338 HIV-positive individuals in Dhaka reveals a critical public health crisis, with over 62% of respondents suffering from severe depression. The research identifies that younger age (18–30), being male, and having a low monthly income are significant predictors of depressive symptoms, while adjusted models highlight that unmarried status and Hindu religion further increase the odds of severe depression. By establishing that depression significantly hinders treatment adherence and quality of life, the study provides a vital mandate for the Ministry of Health to integrate routine psychiatric screening and mental health programs into national HIV care guidelines, ensuring that the "fear to care" shift protects the most vulnerable survivors in Bangladesh.
