Impact statement: This is the first multicentre study to assess prehospital delay in Bangladeshi stroke patients, revealing that an alarmingly 87.72% of patients arrive late (more than 4.5 hours after symptom onset). By identifying that referral delays (median 14 hours) comprise the majority of this time loss and that factors like self-medication, previous stroke history, and lack of specialized transport significantly increase delay, the study provides a critical mandate for mass public information campaigns and the establishment of dedicated stroke centres to ensure patients receive quality care within the optimum therapeutic window.
