| Impact statement: This case report details the diagnostic complexity of a 54-year-old male whose Adult-onset Still’s disease (AOSD) was initially mistaken for infectious diseases like tuberculosis or sarcoidosis. The patient’s rapid deterioration into hemophagocytic lymphohistiocytosis (HLH)—a severe "cytokine storm" with a predicted probability of over 99%- resulted in a fatal outcome despite aggressive treatment.
