Impact statement: This case report details the diagnostic complexity of a 54-year-old male whose Adult-onset Still’s disease (AOSD) was initially mistaken for infectious diseases like tuberculosis or sarcoidosis. The patient’s rapid deterioration into hemophagocytic lymphohistiocytosis (HLH)—a severe "cytokine storm" with a predicted probability of over 99%- resulted in a fatal outcome despite aggressive treatment. By highlighting these diagnostic pitfalls, the study provides a critical mandate for early recognition and a multidisciplinary approach to managing rare autoimmune conditions that mimic sepsis in clinical settings.
