Impact statement: This article documents the first reported case of COVID-19-related phantosmia (olfactory hallucinations) in Bangladesh, involving a 32-year-old physician who experienced persistent phantom odors of onion and garlic. While the condition was brief and resolved spontaneously, it significantly affected the patient's quality of life and ability to eat. The findings highlight that qualitative olfactory changes can occur as late-onset symptoms after recovery, underscoring the need for clinicians to be aware of these vague neurological symptoms to avoid misdiagnosis and ensure appropriate patient support.
