Impact statement: This mixed-method study identifies that road traffic injuries in high-risk zones of Bangladesh are primarily driven by avoidable driver behaviors (lack of sleep, speeding, and ignoring rules) and unplanned infrastructure (broken roads and unplanned curves). A key finding is that while nearly two-thirds of victims have some knowledge of driving rules, passengers are significantly more knowledgeable than drivers, and the presence of such knowledge is directly associated with less severe injuries. These findings provide a vital evidence-based mandate for policymakers to implement stringent traffic enforcement, improve road design, and provide targeted safety education to mitigate the staggering economic loss and health burden caused by road crashes.
