Impact statement: This cross-sectional study of 497 hypertensive adults in northern Bangladesh reveals that one-quarter of patients (27%) initially consult informal healthcare providers, with lower socioeconomic status, rural residency, and younger age being significant predictors of such behaviour. By identifying fear of stroke (45.1%) as the primary motivator for seeking specialized care and establishing that registered physicians diagnosed over half of the cases, the research provides a critical mandate for affordable specialized hypertension centres and community-based awareness programs to ensure patients transition from informal care to evidence-based clinical management.
