Impact statement: This case report documents a rare congenital vascular abnormality—left internal jugular vein (IJV) agenesis—in a 32-year-old man who suffered from a dull, persistent headache for over 20 years. Affecting only 0.05% to 0.25% of the population, such developmental venous anomalies are typically asymptomatic but can lead to impaired cerebral blood flow and intracranial hypertension. By identifying that the patient's long-term pain was linked to this anomaly rather than tension headaches or migraines, the study provides a vital mandate for healthcare workers to remain aware of venous outflow disturbances as a secondary cause of chronic pain. Furthermore, early detection is critical to avoid catastrophic injuries during medical procedures such as central venous cannulation or neck dissections.
