| Impact statement: This case report documents a 77-year-old male in Bangladesh who developed acute encephalopathy and non-ST-segment elevation myocardial infarction (NSTEMI) within 48 hours of receiving the Moderna (mRNA-1273) COVID-19 vaccine. The patient, who had existing comorbidities, exhibited significantly high inflammatory markers (CRP, ESR, D-dimer) shortly after vaccination.
