Impact statement: While global vaccination strategies have successfully reduced maternal and neonatal tetanus, adult men in low- and middle-income countries remain disproportionately vulnerable, accounting for over 73% of adult tetanus cases. This vulnerability is driven by a lack of booster doses in national schedules and high occupational exposure in sectors like agriculture and industry. By identifying that adult tetanus carries a high case fatality rate of 43.2% due to the scarcity and high cost of critical care, this research provides a vital evidence-based mandate for policymakers to implement WHO-recommended booster schedules for boys and men. Such strategies are essential to prevent thousands of avoidable deaths and protect families from the devastating social and economic consequences of the disease.
