Impact statement: This unusual case report describes a 16-year-old child in Bangladesh diagnosed with nasopharyngeal carcinoma (NPC) that rarely metastasized to the orbit (eye), eventually leading to complete visual impairment and severe anaemia. While NPC typically spreads to the bones, lungs, or liver, this case highlights the aggressive nature of the disease in younger patients and the critical need for pretreatment laboratory profiles to predict prognosis. By documenting the complications of chemoradiotherapy and the impact of chronic bleeding on life expectancy, the report provides a vital mandate for clinicians to maintain high surveillance for ocular symptoms and manage nutrition and blood levels to improve the quality of life for paediatric cancer survivors in resource-limited settings.
