| Impact statement: This case report describes an unusual presentation of a 22-year-old woman experiencing severe acute watery diarrhoea specifically tied to her menstrual cycle for three years. By exploring the potential link between increased prostaglandin levels and gastrointestinal symptoms, the study identifies a critical need for clinicians to investigate hormonal fluctuations and medication use (such as norethisterone acetate) in women with persistent menstrual-related GI distress.
