Impact statement: Urinary tract infections (UTIs) are a major cause of morbidity in children, yet treatment guidelines for developing countries are often lacking, and the rise of antibiotic resistance is rapidly limiting oral treatment options. This systematic review of six randomized controlled trials across 13 LMICs reveals that while newer drugs like cefadroxil show promise in preventing recurrence, there is a critical shortage of high-quality, large-scale clinical data specifically from these regions. By identifying that current treatments are often started empirically without local susceptibility data, the study provides a vital mandate for policymakers to invest in regional microbiological research and standardized clinical trials to protect children from long-term renal complications and the global threat of multidrug-resistant infections.
